NDIS Participant Intake Form

This Intake Form was developed via a workshop with 49 NDIS providers facilitated by Gina Ingrouille. The intake form is designed to ask ‘eligibility for service’ and ‘good fit’ questions.  The intention is to ensure providers don’t ask more than is needed at this first ‘info collection’ touch point. Just enough to know if the person is a good fit.

Asking too much information is a waste of everyone’s time and can be traumatising and exhausting for participants and families.

This form is also designed to be your referral form.
